Job details:
- Board Certified/Board Eligible with geriatric experience
- M-F 8a - 8p 8a-8a (part of 1 week a month): One week a month. Weekday 5p-8a
- Duties: Annual assessments, Post-hospital visits, Acute visits
- MA license
- EMR: TruChart
