Job Details

• BC/BE Family Medicine or Internal Medicine


• HIV experienced required


• 13-15 patients per day


• complex care, seeing patients 16 and up


• Experience in population health management and working with psychosocially and medically complex patients is strongly preferred


• provide in-person and telehealth primary/HIV care


• Integrated care team including physicians, nurse care managers, social workers, behavioral health specialists, pharmacists, and community outreach workers
