Description - Join a value-based care organization focused on providing compassionate, patient-centered primary care to Medicare patients in a collaborative team environment. - Board Certified Nurse Practitioner - Must speak Hattian Creole - Full-time outpatient NP role - Medicare patient population - Dedicated Medical Scribe support - Reports to Center Medical Director - Provide routine and acute primary care visits - Conduct annual wellness visits and HRAs - Coordinate care with specialists and care teams - Support population health and quality initiatives - Patient education and care planning - Active NP license and DEA - 2 years of primary care outpatient experience - HCC / ICD-10 / HRA familiarity - Pay range $104,442.00 - $224,952.00 - Full benefits package - PTO * Some information such as the email and phone number are hidden unless you are registered and logged-in
