Licensed Vocational Nurse (LVN/LPN) – Long Term Care – Claremont, NH

What You’ll Do


  • Monitor residents’ mental and physical well-being continuously and communicate changes to the supervisory team

  • Accurately transcribe and implement physician’s orders, administer medications, and document via electronic health records

  • Supervise and support Nursing Assistants, ensuring quality resident care in compliance with licensing standards

  • Assist with resident rounds, facilitate communication with physicians, and support medical needs assessments

  • Administer treatments such as oxygen administration, wound care, and feeding procedures in accordance with facility policies

  • Coordinate resident admissions, discharges, and transfers, ensuring proper documentation and communication

  • Engage with resident families to address concerns and obtain consent as needed

  • Manage shift responsibilities, including reporting, documentation, and staff supervision in a team-oriented environment

  • Follow all regulations related to medication management, infection control, and unit safety

  • Assist with staff orientation and ongoing education to promote continual improvement



What You Need


  • Valid LVN or LPN license in New Hampshire

  • At least 1 year of experience in long-term care or similar healthcare setting

  • Strong communication, organizational, and teamwork skills

  • Ability to work independently and manage multiple responsibilities effectively

  • Reliable transportation required due to rural location and limited public transit options
