What You’ll Do
- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders
- Develop and implement individualized care plans following regulatory and clinical standards
- Perform rehabilitative treatments including ADL retraining, therapeutic activities, exercises, modalities, and assistive devices
- Assess patient progress and modify interventions as needed
- Evaluate treatment outcomes for effectiveness and document appropriately
- Provide counseling and education to patients, families, and caregivers
- Communicate regularly with the healthcare team regarding patient progress and challenges
- Support supervision and mentorship of assistants, aides, or students
- Maintain thorough, compliant documentation of all services and treatments
What You Need
- Valid Occupational Therapist license and NBCOT certification
- At least 1 year of experience in long-term care settings preferred
- Strong knowledge of clinical evaluation, intervention techniques, and documentation procedures
- Excellent communication skills and ability to work within a team
- Commitment to ethical practice aligned with AOTA and state guidelines
