This role involves managing a diverse patient panel, conducting sick and well visits, and performing physicals and vaccinations, with an emphasis on chronic disease management and some women's health services, working 8am-5pm shifts.

Responsibilities and Duties

• Manage chronic conditions including hypertension, diabetes, asthma, and COPD.
• Conduct sick and well visits.
• Perform physicals and administer vaccinations.
• Provide women's health services, including pap smears.
