What You’ll Do
- Manage healthcare needs, ensure quality and cost-effective outcomes for patients within Berkshire Medical Center.
- Coordinate and monitor services and resources needed by patients, utilizing a multidisciplinary team approach.
- Review patient status at admission, throughout the hospital stay, and prior to discharge to ensure appropriate level of care.
- Perform transition care planning activities, securing post-hospital care arrangements.
- Collaborate with physicians, families, insurance providers, and external agencies to facilitate patient care.
- Support performance improvement initiatives to enhance hospital operations.
- Document and analyze patient data to aid in decision-making and compliance processes.
What You Need
Required:
- Valid Massachusetts RN license or eligibility
- At least 1 year of relevant RN experience, preferably in case management or hospital settings
- Strong organizational and interpersonal communication skills
- Ability to work independently and handle multiple priorities
Preferred:
- Familiarity with Medicare, Medicaid, and commercial payers
- Knowledge of community resources and care transition strategies
- Computer literacy, including data management and research skills
