Requirements:

Graduate of an accredited Nurse Practitioner program with a Master’s degree in Nursing – [REQUIRED]
Nurse Practitioner Certification by ANCC or AANPCB – [REQUIRED]
Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED]
Unrestricted license to practice as a Physician Assistant in the State of Colorado – [REQUIRED]
Must be eligible for DEA and Medicare/Medicaid provider status — [REQUIRED]
Clean malpractice/license history - [HIGHLY PREFERRED]
Must have no sanctions/exclusions from HHS/OIG or GSA — [REQUIRED]
Must hold BLS (note in "Additional Submission Details" field) - [REQUIRED]
Experience: 2 years of trauma surgery preferred OR completion of surgical fellowship - [REQUIRED]
Nurse Practitioner Certification by ANCC or AANPCB
Basic Life Support (BLS) – AHA or American Red Cross
Unrestricted Physician Assistant license – State of Colorado
