Job Summary:  The Case Management Lead position provides supervision and coordination of the Case Manager Program within the Case Management Department. The Lead is responsible to assist with hiring, completing performance evaluations and competency assessments of all Case Management staff per HCA policy and following the disciplinary process with the assistance of the Director. This position, along with staff designee, provides orientation to new employees. Lead assures that staff stays current with practice guide lines through use of continuing education, training and provision of clinical supervision. It is expected that this position collaborates within the Case Management Department, nurse managers, staff nurses, physicians, ancillary departments and community resources to ensure that a safe, acceptable discharge plan is in place for all patients seen per policy.   Position Responsibilities: Administers various human resource processes and procedures for the Case Managers to include PMP's, coaching, disciplinary action according to policy with assistance from the department director. Assists Director to maintain compliance with HCA, federal, state and other governing bodies such as Joint Commission. This may involve assisting with projects, review and/or developing policies, and conducting periodic review of Case Manager activities to ensure compliance with policies and procedures. Assists with the hiring and onboarding of new employees
