Description Standard Qualifications: Registered Nurse RN Case Manager - PRN - 1 weekend shift monthly Overland Park Regional Medical Center is one of the premiere medical facilities in Johnson County. The hospital is a licensed 343-bed facility offering acute and outpatient medical care services to the Overland Park community and the surrounding areas since December of 1978. The hospital campus features four medical office buildings, two pharmacies and the offices of more than 100 physicians. Responsibilities: RN Case Manager assesses each assigned patient referral to identify discharge planning needs, and collaborates with patient, family, significant other, and members of the health care team to develop and implement discharge plans and perform utilization review functions based on hospital policy and procedure. Assesses, develops, implements and monitors a comprehensive plan of care through an interdisciplinary process; providing both the clinical and psychosocial approach through the continuum of care. Balances individually identified patient and family needs with the efficient and cost-effective utilization of resources.The Case Manager in department 768 provides care for newborn/pediatric patients from birth to 11 years of age; adolescent patients between 12 to 17 years of age; adult patients between 18 to 64 years; and geriatric patients 65 years and older. * Some information such as the email and phone number are hidden unless you are registered and logged-in
