Description Standard Qualifications: TriStar Centennial Medical Center - Nashville, TN Registered Nurse (RN)--Case Management--Full-Time Weekends Only Position: 12-hour shifts on Friday/Saturday/Sunday plus one weekday 8-hr shift every two weeks. Our 657 bed hospital is both nationally and locally recognized, including being named as one of the nation's Top 100 Hospitals in the areas of cardiology, stroke, orthopaedics and breast cancer management. As a result of the continued advances in quality care, many improvements in all areas of healthcare have been made and thousands of lives have been significantly impacted. This position will play a key part in continuing our success! Centennial Medical Center is currently looking for a Registered Nurse (RN)--Case Management. Responsibilities: Demonstrates knowledge of human growth and development, assessment, range of treatment and care of patients appropriate to the ages of patients served as per Scope of Service.Promotes internal and external customer service initiativesUtilizes critical thinking skills to meet needs of individual patients.Demonstrates ability to perform technical skills and procedures.Documents all patient care.Participate in Performance Improvement activities as assigned.Works with Business office to manage payment issues of third party xxxxxxxxxxx*municates with third party payers to review patients' care.Acts as a liaison with patients, physicians, and Program Coordinator to keep them informed of care issues, such as discharge plans, insurance reviews, etc.Actively works with treatment team to assist patient and family with discharge plans. * Some information such as the email and phone number are hidden unless you are registered and logged-in
