Position Summary: This position supports the mission of Presbyterian/St Luke's Medical Center of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Cardiovascular (CV) Nurse Practitioner (NP) / Physician Assistant (PA) exemplifies professional nursing practice through expert education, consultation, research, practice, and management/leadership activities. The CV NP/PA is a leading nurse expert in cardiac and vascular care across all settings, and acts as a resource for, support to, and leader of cardiovascular resources within the hospital. In a collaborative manner, the CV NP/PA leads in the design, implementation, and evaluation of new programs, projects, and systems regarding cardiac and vascular care. The CV NP/PA collaborates with the physicians and interdisciplinary team members to review the plan of care and intervene when necessary to ensure excellent delivery of quality health care in a cost effective way. Utilizing extensive advanced knowledge of pathophysiology and pharmacology, the CV NP/PA exercises independent judgment in the assessment, diagnosis and initiation of certain delegated medical processes and procedures. The ultimate goal of the CV NP/PA is to improve continuity of patient care and facilitate patient flow throughout the health care continuum. The CV NP/PA actively participates in program development for the cardiac service line in collaboration with and under the direction of the Cardiac Service Line Manager and VP of Service Line Operations. Roles and Responsibilities May Include: Perform comprehensive initial History & Physical. Plan for comprehensive pre-operative evaluation and patient/family xxxxxxxxxxx*municate with families and the PICU progress of the patient and the operation.Conduct rounds, in addition to physician's rounds, develop and implement patient management plans.Consult with other providers, including the Emergency Department, anesthesia, general surgery etc, appropriate care for complex cardiac congenital heart patients.Activate approved treatment protocols and obtain orders from medical staff members in compliance with the facility-defined process for annual protocol approval by the responsible practitioner and authentication of each order or set of orders as defined by the medical staff bylaws, rules and regulations. Monitor patient response to treatment and adjust treatment within the guidelines established by the treatment protocol and/or advanced practice act Assist physician with procedures, in accordance with defined qualifications and competencies, based on licensure and related practice act. Perform procedures after discussion with care team and surgeon in accordance with defined qualifications and competencies, based on licensure and related practice act. Collaborate with other healthcare professionals in interdisciplinary rounds for patient care and discharge to facilitate continuity of care and integration of evidence based practice. Conduct facility required congenital cardiac patient and family education. Assemble and analyze patient data to facilitate decisionmaking, patient throughput, and imporved patient care Actively participate in and support congenital cardiac quality and outcomes management initiatives through monthly clinical standards meeting: weekly cardiac surgery conference and quarterly morbidity and mortality conference. Provide congenital cardiac educational programs for employees, physicians or community groups. Support patients and families through outreach to congenital cardiac supported camps (Roundup River Ranch).Assist in scheduling patients based on patient, family and team needs.Follow complex, shunt dependent single ventricle patients once discharged from the facility and communicates significant findings or concerns with cardiologist, PICU attending, and surgeon.Support Facility in ensuring compliance with the Joint Commission, state licensure, CMS and other regulatory agency requirements. Serve as a resource for Facility's employees in the provision of patient care by providing expert advice and
