Description Position: RN Care Coordinator/Clinical Triage Specialist Location: St. Paul, MN Shift: Full Time Job Info Serve as a RN care coordinator to include clinical care plan follow-up, coordinating referrals, work directly with the customer service team to address the needs of our patientsCollaborate and work directly with all clinical and business areas to include navigation team, clinical operations, field staff and other business professionalsCoordinate and interact with key external stakeholders: consumers/pts., community partners, Sr. Clinical Leadership team, payers, providers, vendorsDirect the triage team under the guidance of the Clinical Triage and Navigation Manager, to include, but not limited to: implementing triage protocols, guiding the consumer/pts through evidence-based triage guidelines, helping to set appts, retrieve clinical data and operations, coordinate with provider regarding pts’ plan of care, etc.Provide coverage for dispatch and field staff, when neededRequirements MN state RN license, in good standing; BSN preferred3+ years commensurate experience in an ambulatory or clinical environment, community-based health care, etc.Clinical knowledge and experience that ideally spans the lifecycle continuum, to include pediatrics, adults and elder carePossess strong communication skills-motivational interviewing techniques a plusAbility to think critically and make fact-based decisionsHandle a variety of tasks in a fast-paced environmentBe a problem solver and a doerSelf-starter; ability to work independentlyKnow how to lead and motivate people, pts and teamsFor immediate consideration please create a profile and submit your resume through our applicant tracking system.
