Description Position: RN Supervisor - Hospice Location: San Diego, CA Shift: Full Time Job Info Receives case referralsReviews available patient information related to case, including home visits, to determine hospice/home care needsAssigns appropriate hospice personnel to case as neededConferences with medical director regarding any questions about an individual’s eligibility for servicesReviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by cliniciansDiscusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care servicesIs available at all times during operating hours to assist clinicians as appropriateReviews patient’s medical diagnosis, prognosis, medications, procedures and clinical courseOther duties as assignedRequirements Registered nurse with current licensure to practice professional nursing in the state of California.Graduate of National League of Nursing accredited school of nursing with a BSN degree is preferred or a minimum of one (1) year of experience as a professional nurse within the last three (3) years.Previous experience in hospice/home care setting and with two years management or supervisory experience.
