The Hospice Admissions Nurse uses advanced knowledge and skilled nursing to provide admission and skilled nursing visits in compliance with state Registered Nurse Practice Act. As a member of the program’s clinical team, consistently exercises discretion and judgment to manage all aspects of a patient’s admissions, including performing clinical assessment of patient’s appropriate care needs and admitting the patient if appropriate, coordination and management of care plan development and oversight of care transition.  Working collaboratively with the marketing team, responsibilities include presenting services to patients and their families in a manner that represents our company’s philosophical mission resulting in the admission of patients at the appropriate level of care. This begins with combined efforts in obtaining and communicating information to our patients and/or referral sources. Actively participate in the Interdisciplinary Team as clinical liaison, providing specialized clinical advice and knowledge in an effort to bring excellent Hospice care services to patients and their families. Additional responsibilities include overseeing pending patients by utilizing branch processes, routinely following the progress of prospective patients, clinical communication with referral sources, and provide referral sources ongoing information about our services. Works under the general direction of the Clinical leader.

Completes clinical assessment to identify patient’s care needs. Makes determination if patient should be admitted into Hospice services. Admits patients to the services, completing appropriate clinical document under the Clinical Managers direction.
Coordination and management of care plan development and oversight of care transition, encompassing various healthcare providers.
As a member of the Interdisciplinary team, the Registered Nurse utilizes the nursing process to contribute to each patient’s total plan of
Initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living
Risk for pathological
Cultural and spiritual implications
Verbal and non-verbal communication
Implements the individualized plan of care and recommending revisions to the plan as necessary, managing discomfort and providing system relief.
Provides specialized nursing skills related to palliative and end-of-life care.
Consults with and provides education and advice to the patient/family regarding: The disease process, self-care techniques, End-of-Life Care.
Manages clinical and progress notes that demonstrate progress toward established goal(s). Obtain and record all data related to the referral and admission Prepare charts and develop care plans.
Coordinating all patient/family services and prioritization of needs with the members of the interdisciplinary Use of case management approach and referring to other services as needed.
Supervises LPNs/LVNs and paraprofessionals providing services to the patient according to regulatory
Determines the scope and frequency of services needed based on acuity and patient/family
Provides specialized hospice training to other staff, family members and informal caregivers to insure adequate care
Assesses the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout
Visit patients and families in their place of residence or in the hospital to assess for appropriateness, explain services, monitor condition, or admit to hospice services in accordance with the admission
Provide complete and timely verbal and written communications regarding referrals and the status of potential admissions to the Patient Care Manager and other members of the hospice team as
Assure for compliance with local, state and federal laws, Medicare regulations, and established personnel policies and
Make routine nursing visits as directed
Participate in evening/weekend on-call and overtime as required, conducting on-call services in a competent and responsive
Adheres to and participates in Company’s mandatory HIPAA privacy program / practices, Business Ethics and Compliance programs / practices, and all Company policies and
Participates in special projects and performs other duties as assigned
Registered Nurse licensed in the
Bachelor’s degree and H.P.N. (Certified Hospice and Palliative Nurse) certification preferred.
Minimum of one-year general nursing experience preferred with current Hospice, medical, surgical or critical care
Advanced knowledge of clinical manifestations of diseases and ability to develop hospice-specific clinical
Ability to exercise discretion and judgment in the clinical assessment of patient needs associated with admission and ongoing care.
Understanding of Hospice philosophy, principles of death/dying. G
