Description StarMed has partnered with a large hospital system in Florida to assist with adding Managers of Care Management to their expanding Care Coordination departments within three area hospitals! The Manager of Care Management will lead the case management and care coordination teams in facilitating appropriate use of hospital resources and effective discharge planning and coordination of services to transition patients to the next appropriate level of care. Additional responsibilities include managing the educational, financial and human resource needs for the case management and care coordination teams to deliver high quality, cost effective, and responsive patient-focused health care services; oversight of the workload distribution and scheduling of the team members to assure appropriate staffing to meet the needs of patients and families within the Orlando Health system; development of collaborative relationships with the site administrators, medical staff leadership, managed care contractors, and community leaders. Five (5) years of related care coordination experience to include two (2) years of experience in a leadership role.
