-Duration: July – December,

-Location: Toledo, OH : Telehealth – visits from home (option for in clinic if preferred)

-Schedule: Part-time or full-time per your availability (2-5 days per week)

-Shift: Monday – Friday, 8a-5p

-Call: None

-Scope: Outpatient primary care

-Patients: Age range per your comfort level, NP supervision

-Volume: 15-18 patients per day

-EMR: All Scripts

-Required: Ohio license, Ohio DEA, Enrolled in Medicare and Medicaid
