Location: Phoenix, AZ

Dates: ASAP - Ongoing

Schedule: Monday - Friday, 8:30-5 

Scope: Adult primary care - Comfortable with well and sick visits for homeless and undeserved patients, self-sufficient, team oriented, strong charting skills.

Volume: Approximately 25 patients per day

Support Staff: 1MD, 1 Mid-level, Nursing support, CNA support

Required: Active AZ license, 2 years experience as an APP required.
