Description Duration: October – February Reason for coverage: Maternity leave and Sabbatical Schedule: Really needs someone there every week (will consider 4 days per week) Shift: Monday – Friday, 8a-5p (no weekends, no extended hours) Call: None Patient volume: 8-10 patients per day Scope: Adult outpatient, integrated comprehensive practice model (no crisis prevention) EMR: Epic Hospital: Community Health Hospital in town helps with any crisis management Group/support: IM, FP, PED, PSY, LCSWs Requirements (boards, license): Oregon license required at time of application and board certified or board eligible * Some information such as the email and phone number are hidden unless you are registered and logged-in
