Location: Eugene, OR
Duration: July 2022 – July 2023 (1yr contract – potential to extend) or perm transition.
Reason for coverage: re-open clinic.
Schedule: Monday – Friday 9-5.30 *6-hour per day – hours based on provider.
Call: N/A
Patient volume: 4-8 patients per day. Picking up – new location. (30 min appointments for now)
Scope: outpatient primary care with heavy BH: ADD/ADHD, personality disorder, anxiety/depression
EMR: NextGen
Hospital: walk in clinic – no appt
Group/support: 12 exam rooms: PC team – 1 provider, MA, 1 scribe + nursing staff: walk-in triage – everyone is trained in de-escalation
Requirements (boards, license): FP/IM – BC, OR licensed + covid vaccine + boosters (per OR bylaws
