- 24 hour call: 7:00 am - 7:00 am
- Exact Dates:
- June 12 @ 7:00 am - June 21 @ 7:00 am
- July 17 @ 7:00 am - July 26 @ 7:00 am
- Aug 21 @ 7:00 am - Aug 30 @ 7:00 am
- Clinical Information:
- Emergency Department and Hospital consults and emergencies
- Common Diagnoses/Procedures:
Hydronephrosis with renal and ureteral calculous obstruction, Calculus of kidney,Benign prostatic , hyperplasia with lower urinary tract symptoms, Calculus of ureter,Urinary tract infection
- Patient volume: 0-10 daily
- Only 1 full time Urologist in FPN
- Need additional coverage for Hospital not for the office
- Only ED and Hospital work, no office
- Cover emergencies and any needing surgery
Required:
- Must Use EPIC
- Active IN license, DEA and CSR
- Clean file to start in June
- CV must be in Month / Year format
- Must have license in hand at time of submission, or proof that the license application has been submitted
- Any gaps greater than 30 days will require an explanation on the CV
- DEA must remain in state to maintain privileges.Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Current Flu Vaccine
- Must be fully vaccinated for Covid 19
- Exemptions only allowed for religious or medical reasons, once approved by the facility.
