Requirements

• Board Certified
  
• ACLS/BLS
  
• Provider must be local
  
• Open to three cites
  
• Fellowship - trained
  
• Scope of Services Required: Adult Cardiac Surgery, General Thoracic Surgery
  
• Aortic Surgery Procedures Required: Descending Aortic Aneurysms
  
• Coronary Artery Bypass Surgery Procedures Required: CABG, Arterial Revascularization (Using Internal Mammary Arteries, Radial Artery and Gastroepiploic Artery), Redo Coronary Revascularization (With or Without Heart Valve Surgery), Off-Pump CABG, Pericardial Window, Thoracic aortic aneurysms, Thoracoabdominal aneurysms, Tracheostomy
