To maximize these efforts for our Reserve Members, QTC will be holding “RHRP Events” on the military bases. We are URGENTLY looking for providers in the area to attend and perform the physical exams needed. These events will typically take place on a weekend, and we would give you plenty of notice before hand. Conveniently, if you are not able to attend a certain event, you can respectfully decline until there is an event date in which you are able.

Separation History and Physical Exam (SHPE)

DoD Periodic Health Assessment (PHA) with Mental Health Assessment (MHA)

DoD Periodic Health Assessment (PHA) Lite

Post-Deployment Health Reassessment (PDHRA)

U.S. Marine Corps Forces Reserve (MARFORRES)

Active Components (AC) Army TRICARE Prime Remote (TPR) / Army National Guard (ARNG) / U.S. Army Reserve (USAR), Pre-Defense Health Agency (DHA)

Provider Qualifications
Only Physicians, with a full, clear, active and unrestricted state license as Doctor of Medicine (M.D.) or Doctor
of Osteopathic Medicine (D.O.), or Physician Assistant, (P.A.), or Nurse Practitioner (N.P.), or Advance Practice
Nurse (APN) who have been pre-paneled and pre-approved by QTC may perform Medical Examination
Services including the review of medical records (when applicable), conduct history and examination, and
interpret all clinical data when applicable. No other persons, including medical staff, technicians, therapists, and
colleagues/physicians that are not pre-paneled and pre-approved by QTC are authorized to participate in the
history and examination process. Upon any change in certification status, Contractor agrees to notify QTC
immediately but always prior to conducting the RHRP examination.
