What You’ll Do
- Monitor residents’ mental and physical well-being continuously and communicate changes to the supervisory team
- Accurately transcribe and implement physician’s orders, administer medications, and document via electronic health records
- Supervise and support Nursing Assistants, ensuring quality resident care in compliance with licensing standards
- Assist with resident rounds, facilitate communication with physicians, and support medical needs assessments
- Administer treatments such as oxygen administration, wound care, and feeding procedures in accordance with facility policies
- Coordinate resident admissions, discharges, and transfers, ensuring proper documentation and communication
- Engage with resident families to address concerns and obtain consent as needed
- Manage shift responsibilities, including reporting, documentation, and staff supervision in a team-oriented environment
- Follow all regulations related to medication management, infection control, and unit safety
- Assist with staff orientation and ongoing education to promote continual improvement
What You Need
- Valid LVN or LPN license in New Hampshire
- At least 1 year of experience in long-term care or similar healthcare setting
- Strong communication, organizational, and teamwork skills
- Ability to work independently and manage multiple responsibilities effectively
- Reliable transportation required due to rural location and limited public transit options
