Description
• Location: Multiple locations in South/Central Chicago, IL
• Duration: ASAP-Ongoing (3–6-month locum contract w/ option to extend)
• Schedule: 3-4 days per week – option for hybrid telemedicine/on-site or 100% onsite
• Shift: Mon-Fri (8a-5p) – flexibility to make your schedule
• Setting: Hybrid Telemedicine & Outpatient Psychiatry. Integrative care approach with FQHC Community clinic where Psychiatrist will treat psychiatric/behavioral conditions and support the Primary Care and Behavioral Health Manger
• Patient Volume: 14-16
• EMR: Athena
• Credentialing: 30-45 Days
Skills
• Requirements: Board certified/eligible, IL License
• Prefer Bilingual English/Spanish and local candidates
